What Are Acne Scars?

Acne scars develop due to inflammation in the skin's inner layer, the dermis, triggered by acne. This inflammation occurs when skin pores become clogged with excess oil, bacteria, and dead skin cells, leading to acne breakouts. In some instances, this inflammation can harm the skin tissue, leading to the formation of scars.

Types of Acne Scars

Acne can result in seven different types of scars, and it's common to have more than one type. If you have acne-prone skin, you may observe:

Ice-pick acne scars: Ice-pick scars look like tiny, deep holes in the skin as if someone poked them with an ice pick. They often show up on the forehead or upper cheeks because the skin there is thinner.

Boxcar acne scars:  Boxcar scars may resemble the impression of a square or circle on your skin. These types of scars are often observed on the jawline or lower cheeks.

Rolling acne scars: If your skin looks uneven and wavy, you might have rolling scars. These scars result from damage beneath the skin's surface and are often seen on the lower cheeks or around the jawline.

Hypertrophic acne scars: Certain scars can create a depression in the skin, whereas others, such as hypertrophic scars, lead to the formation of either flat or raised scar tissue in the area where acne used to be. This happens due to the excessive production of collagen over the affected region. These particular types of acne scars can manifest on various body parts, including the face, chest, back, or shoulders.

Keloid acne scars:  The raised bumps may appear darker than your skin tone and could be larger than the original acne lesion. Like hypertrophic scars, keloid scars can sometimes be itchy or uncomfortable.

Dark spots or staining and acne scars: Once a pimple has cleared up, you might spot a dark mark, especially if your skin tone is medium to dark. This happens because acne can trigger your skin to produce more melanin (pigment).

Perifollicular elastolysis: Small round or oval bumps that may blend in with your skin tone are known as papular scars. They can be found on various body parts such as the arms, chest, back, shoulders, and even the face.

Acne Scar Causes

Acne can leave scars on your skin due to several reasons:

  • Acne can lead to long-lasting scars, especially when it penetrates deep into the skin. Nodules and cysts, which are solid lumps and pus-filled lesions, are more likely to cause scarring.
  • Picking or squeezing pimples is a common habit, but it can worsen the situation. Irritating a pimple by picking at it increases inflammation and the risk of scarring, rather than helping it heal faster.
  • Your skin's healing process also influences scarring. Excessive collagen production can lead to raised scars, which are more common in people with dark skin. Conversely, insufficient collagen can result in sunken acne scars.
  • Genetics play a role in acne scarring; if a family member has had acne scarring, you may be more susceptible to it. Additionally, having darker skin increases the likelihood of developing scars.

Symptoms of Acne Scars

1. Depressed or raised skin areas

2. Skin discoloration (hyperpigmentation or hypopigmentation)

3. Rough or uneven skin texture

4. Redness or inflammation

5. Itchiness or discomfort

Treatment for Acne Scars

  • Topical treatments: Over-the-counter creams and gels containing ingredients like retinoids, vitamin C, or alpha hydroxy acids can help improve mild acne scars by promoting skin cell turnover and collagen production.
  • Laser therapy: Different types of lasers, such as ablative and non-ablative lasers, can help improve the appearance of acne scars. They work by stimulating collagen production and resurfacing the skin.
  • Chemical peels: This treatment involves applying a chemical solution to the skin to exfoliate the outer layer, revealing smoother skin underneath. Chemical peels can improve the appearance of mild to moderate acne scars.
  • Microneedling: Also known as collagen induction therapy, microneedling involves using a device with fine needles to create tiny punctures in the skin. This process stimulates collagen production, which can help improve the appearance of acne scars.
  • Dermal fillers: Fillers containing substances like hyaluronic acid can be injected into acne scars to temporarily plump them up and make them less noticeable. This is a temporary solution and may need to be repeated periodically.
  • Steroid injections: For raised or keloid scars, corticosteroid injections can help reduce inflammation and flatten the scar tissue.
  • Surgery: In some cases, surgical procedures like punch excision or subcision may be used to treat deep acne scars. These procedures involve removing the scar tissue or breaking up the scar tissue to improve the skin's appearance.

Acne Scar Prevention

To reduce the risk of scarring, consider these tips:

  • Avoid picking, popping, or squeezing acne
  • Keep your hands away from your face
  • Use mild cleansers
  • Quit smoking
  • Avoid tanning
  • Pay attention to your diet
